Open An Account

Click below and print the form to help you start the account opening process.  Once you have completed the Deposit Account Application, please bring it to any Citizens Bank of Florida office.  This will save time in the account opening process and ensure that you have the required documentation. This form cannot be electronically submitted.  All new accounts will be subject to a Chex-Systems Verification and all required disclosures will be given to you in traditional hard copy format prior to account opening. 

  • For Personal accounts, acceptable forms of Identification to open an account are outlined on the form below.
  • For Business accounts, please call 407-365-6611, for the documents required to open your account. 
  • If you plan to open any type of Trust account, you must bring a copy of the trust agreement with you.

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all banks to obtain, verify and record information that identifies each person who opens an account.  What this means for you:  When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you.  We may also ask to see your driverís license or other identifying documents.
